How to register:

Completed forms should be sent with payment to: the CDI, 18 Jeffries Passage, Guildford, Surrey, GU1 4AP, UK or faxed to +44 1483 567042. Fee does NOT include accommodation.
For security reasons, we recommend that you do not email this form.
Programme Materials: Participants will be given all conference papers in an electronic format (online access). If you are unable to attend but would like to purchase these materials at £175 Sterling, please indicate this on the registration form or call Isabelle Porri.
Admittance & Payment: Only pre-registered, pre-paid registrants will be guaranteed access to the conference. On-site registrants with payment will be admitted on a space available basis. Payments should be made by Visa/MasterCard/American Express or by direct transfer to our account.

DELEGATES SHOULD MAKE THEIR OWN RESERVATIONS DIRECT WITH THE HOTEL USING THE LINK PROVIDED. DO NOT SEND HOTEL RESERVATIONS TO THE CDI.
Cancellations: Registration fees are refundable subject to a 10% service charge if cancellation is made by 28 April 2017, No refunds will be made after that date. All cancellations must be made in writing. Substitutions from the same company may be made at any time at no charge. The CDI reserves the right to cancel any conference it deems necessary and will in such event make a full refund of the registration fee. Any other refunds will be made at the discretion of the management.
No liability is assumed by the CDI for changes in programme content.
