Completed forms should be sent with payment to: the CDI, 167 High Street, Guildford, Surrey, GU1 3AJ, UK. Fee does NOT include accommodation.
For security reasons, we recommend that you do not email this form. Please use the downloadable form if paying by Amex, you cannot register online with Amex.
Programme Materials: Participants will be given a set of conference papers. If you are unable to attend but would like to purchase these materials at £175 Sterling, please indicate this on the registration form or call Isabelle Porri.
Admittance & Payment: Only pre-registered, pre-paid registrants will be guaranteed access to the conference. On-site registrants with payment will be admitted on a space available basis. Payments should be made by Visa/MasterCard/American Express or by direct transfer to our account.

We have secured special rates at the hotel – see “Hotel” page for details. DELEGATES SHOULD MAKE THEIR OWN RESERVATIONS DIRECT WITH THE HOTEL. DO NOT SEND HOTEL RESERVATIONS TO THE CDI.
Cancellations: Registration fees are refundable subject to a 10% service charge IF CANCELLATION IS MADE BY 14 September 2010. NO REFUND WILL BE GIVEN AFTER THAT DATE. All cancellations must be made in writing. Substitutions from the same company may be made at any time at no charge. The CDI reserves the right to cancel any conference it deems necessary and will in such event make a full refund of the registration fee. No liability is assumed by the CDI for changes in programme content.
